In a forum I admin, to help stem the tide of spam bots, we created a few 'custom' questions/answers that the user must answer before they can register.
In this case, we could ask something Morrowind specific and something generic.
"Are you human?" - default "No", "Maybe", "Yes"
"What company released Morrowind?" - default "Bioware", "Microsoft", "Bethesda"
Giving three possible answers helps muddy the waters a bit.
Failing 3 times to answer the questions put a temp ban on their IP for a 30 minutes. "Retry in 30 minutes please."

As I recently suggested (and probably got ignored for the time being because that server owner mostly comes on weekends) on another forum that is being spammed by bots, you could do it as a mini IQ test (requiring IQ>=20 && very minimal common sense).
Something like:
"Using the following table, what would be the color of the Sun?" I imagine captcha bots would enter "From To", and not the right answer.

Actually that may be easy because you can just have the bot look for a specified Hexadecimal color and Click on it. More like a picture of a girl holding a basketball or someone jumping and asking what is this person doing, or what is this person holding?
